(b) One-Time Payment Information: When you make a one-time payment through the Site, we may collect (a) your PaymentsMD account number; (b) first and last name; (c) address; and (d) payment information (e.g., credit card data) so our third-party vendor can process your payments. Note, we do not store your credit card information, however we will retain authorization codes for the transactions you request.
(c) Information to Process Your Billing Information Requests: If you request billing information for a patient in our database, we may request (a) the patient’s social security number; (b) your federal tax ID; (c) your email; (d) your response to a security question.
(d) User Login Attempt Information: We use our web server, to gather information from your browser, including IP address and username so that we may monitor failed login attempts on the Site.
(e) Tracking Technologies To Gather Technical Usage Information: We may use tracking technologies such as cookies and other similar technologies to measure usage. For more information about cookies and similar technologies that may be used on our Site please review section 7 of this Privacy Notice.
Uses and Retention
We use information collected from you to process payments and provide billing information as you request. Your information is retained for as long as we need it for our business.

We may share your personal information and other information we collect with the following entities: (a) ApolloMD Business Services, LLC ("ABS"); (b) ABS's and PaymentsMD's affiliates and subsidiaries; (c) ABS's and PaymentsMD's service providers (e.g., Moneris Solutions, Inc., insurers, attorneys and healthcare providers); (d) entities for which PaymentsMD performs billing services; (e) other entities with my consent; (f) third parties in response to a subpoena, legal request, request made through the paymentsmd.com website, legal and law enforcement; and (g) others if there is a change of control.

If you register for the payment portal, you choose to give us your personal information and are not anonymous to us. When you register for the patient payment portal you may provide personal information. Personal information is only associated with your payment portal account when you sign in as a registered user.
(b) One-Time Payments: When you make a one-time payment through the Site, we may collect (a) your PaymentsMD account number; (b) first and last name; (c) address and (d) payment information (e.g., credit card data) for processing your payment requests. Note that we do not store payment/credit card data, however, we will retain authorization codes for the transactions you request.
(c) Information to Process Your Billing Information Requests: If you request billing information for a patient in our database, we may request (a) the social security number for the patient; (b) your federal tax ID; (c) your email; and (d) your response to a security question.
(d) User Log-in Attempt Information: We use our web server, to gather information from your browser, including IP address and username to monitor failed login attempts to anticipate and prevent interferences with user access to our Site.
(e) Tracking Technologies to Gather Technical Usage Information: When you visit our Site, we may from time to time collect usage information sent to us by your computer, mobile, or other access device that tells us how you are using the Site (“usage information”). We may use tracking technologies (including cookies (including cookies stored in HTML-5), web beacons/GIFs, embedded scripts, e-tags/cache browsers) to collect usage information.
Our goal is to collect and use only that information that we think is required for our legitimate business interests, in order to better understand your interests and improve your experience of using the Site. (b) Analyze and Aggregate Non-Personal Information: We use aggregate information about our users and non-personal information to analyze Site and user behavior as it relates to access and log-in attempts.
(c) Prevent Fraud: We use information from the Site to prevent, detect, and investigate fraud, security breaches, potentially prohibited or illegal activities, and enforce our Terms of Use.
(d) Communicate With You: We use your information to contact you to maintain the Site, your account, to provide other services requested by you, as described at the time when we collect the information from you, or contact you for other purposes authorized by law.

The information may be disclosed, among other reasons, to: (i) process payments; (ii) facilitate Site registration; or (iii) help detect and prevent potentially illegal acts, violations of our policies, fraud and/or data security breaches.
(b) Service Providers and Other Third-Parties: We may disclose your information to service providers under contract who help with our business operations (such as, but not limited to, payment processing, e.g. Moneris Solutions, Inc., insurers, attorneys and healthcare providers). We may also share your information with other third-party entities for which we provide billing services.
(c) Other Entities with Your Consent: Other third parties to whom you explicitly ask us to send your information (or about whom you are otherwise explicitly notified and consent to when using a specific service).
(d) Legal and Law Enforcement: Law enforcement, governmental agencies, or authorized third parties, in response to a verified request relating to a criminal investigation or alleged illegal activity or any other activity that may expose us, you, or any other PaymentsMD user to legal liability. In such events, we will only disclose information relevant and necessary to the investigation or inquiry, such as name, city, state, zip code, telephone number, email address, user ID history, IP address, fraud complaints and anything else we may deem relevant to the investigation.
(e) Change of Control – New Owners: We may share your information with other business entities, should we plan to merge with or be acquired by that business entity. Should such a combination occur, we will make reasonable efforts to request that the new combined entity follow this Privacy Notice with respect to your personal information. If your personal information would be used contrary to this policy, we will request that the new owner provide you with prior notice.
(f) Companies in The Mobile App Industry: If you access the Site through a mobile device, certain browsing information may be automatically shared with carriers, operating systems and platforms.
(g) Third Parties Pursuant to a Subpoena or Other Request: We may share your information with third parties in response to a subpoena, other legal process and/or as permitted by law.

CookiesA cookie (sometimes referred to as local storage object or LSO) is a data file placed on a device. Cookies can be created through a variety of web-related protocols and technologies, such as HTTP (sometimes referred to as “browser cookies”), and HTML5.
Web BeaconsSmall graphic images or other web programming code called web beacons (also known as “1×1 GIFs” or “clear GIFs”) may be included in our Site’s pages and messages. Web beacons may be invisible to you, but any electronic image or other web programming code inserted into a page or email can act as a web beacon.
Clear GIFs are tiny graphics with a unique identifier, similar in function to cookies. In contrast to HTTP cookies, which are stored on a user's computer hard drive, clear GIFs are embedded invisibly on web pages and are about the size of the period at the end of this sentence.
Embedded ScriptAn embedded script is programming code that is designed to collect information about your interactions with the Site, such as the links you click on. The code is temporarily downloaded onto your device from our web server or a third-party service provider, is active only while you are connected to the Site, and is deactivated or deleted thereafter.
ETag, or entity tagA feature of the cache in browsers, an ETag is an opaque identifier assigned by a web server to a specific version of a resource found at a URL. If the resource content at that URL ever changes, a new and different ETag is assigned. Used in this manner ETags are a form of device identifier. ETag tracking may generate unique tracking values even where the consumer blocks HTTP, Flash, and/or HTML5 cookies.
